Chattering/clattering and lack of power

My car lacks power on acceleration, at highway speeds and going uphill. It also makes a clattering/chattering sound when under load, but it goes away at speed (running even over flat ground) or if you meter the gas pedal enough and accelerate /very/ carefully and slowly. Fluids are clean and changed regularly, and the car idles very well. What could be causing something like this?

Could have a clogged cat when the car has sat over night and cooled reach under the car and smack the cat with your hand.See if the cat is rattling.
