1999 Honda Accord has no brake pressure pads and shoes are good what else can it be?

The brakes have not been worked on lately but pads and shoes are in fair condition. you must press way to hard to make the car stop what else can be wrong besides the obvious normal wear?

Rear brakes out of adjustment. Air in lines. Leak in system.
