How/where does the cable bracket for the exterior 60/40 door handle attach (latch end not handle end) attach

I have a 1999 ford e150 cargo van. The cable bracket for the exterior door handle has come loose (bracket looks to have a broken spot weld on it) and the cable (with bracket still attached) is dangling in the door cavity. I cannot find where the bracket would have been mounted, nor where the cable's ball end would connect. I intend to bore out weld and put bolt in it's place, if I can find where it does attach. I went to the Ford dealership (who volunteered to give me a copy of the diagram for the assembly) but found I had the wrong diagram when I got home.
