dual fan relay ..??

I installed dual fan in my 1999 tahoe in this way

How can i start one fan by temp switch and AC ?
Can i use 5 pin relay to do it? and how ?

Do you have a working on/off temp switch in the engine to operate it from?

Yes of course i have temp switch .

I mean a switch, not a temp sensor. The engine didn't come with one.
