Noise when idling and putting clutch down. Sounds like thrust bearing may be gone and I cannot get 5th gear at all. Should I buy a full replacement clutch kit and a secondhand gearbox and be done with it?

If you can't get 5th gear, it sounds like a transmission issue and not the clutch. However, if you take it apart to replace the transmission, I would put a clutch kit into it.

I would suggest taking it out from the bottom.

So the transmission can be taken out without having to remove the whole engine?

The only recommendation I can give you is to get a good repair manual from a parts store. That should provide all the directions.

They offer Mitchel online through the webpage, but you would have to look at it to see if it will help.
