I have a 98 caravan and when I take off from a stop I have to pritty much floor it to get it to move but when I get it goin it smooths out then I have to do this over and over every stop I havnt driven it in bout a week cause of it how can I fix it or can i

First need to determine if it is the engine or trans. Does the trans shift correctly?
Check fuel pressure first on motor then check back pressure on cat converter.
