My front brakes make noise while braking and the disc has a slight hole in it, what could be the cause of the noise?

There are lots of things experienced mechanics do to prevent different noises during a routine brake job, and there are many things one can do to unintentionally cause noises. With so many possibilities, you have to describe it more completely, as in when does it occur, how loud, during braking or all the time, etc. Describe that hole too. Is it a rust hole or a manufactured hole?
