1997 Eagle Talon power steering belt problem

1997 Eagle Talon - I've had the power steering belt replaced over 3 times in the last 3 months in addition to several times in the last 2 years, also most recently had the tension pulley and power steering belt replaced. It only lasted 10 days before belt came off and separated, had the belt replaced again and it came off and shredded again in less than 24 hrs. Any ideas would be appreciated.

Check the pulley conditions. They should be smooth and without any indents.

Check belt alignment. They should be in a straight line and if the power steering pump or tensioner bearing are not aligned corectly, the belt would be be pushed out as the pullies would be tilting to one side.
