How often should I service the radiator

My car is starting to overheat on hot days, but is fine when the days are cooler?

So how do you know its overheating?Is it boiling over the cap?Also are both the upper and lower radiator hoses getting hot?Is your radiator fan working correctly?Any codes in the computer?
