Why does my car smoke during rough idle in gear only?

95 Crown Vic w/ 4.6. Car runs smooth in park. When in gear with brake on there is intermittent rough idle and blue smoke from tailpipe. What should I look for? Car has 140xxx miles. I can rev engine in park with no smoke. Engine idles smooth in park also.
