Why does my car smoke during rough idle in gear only?

95 Crown Vic w/ 4.6. Car runs smooth in park. When in gear with brake on there is intermittent rough idle and blue smoke from tailpipe. What should I look for? Car has 140xxx miles. I can rev engine in park with no smoke. Engine idles smooth in park also.

Check for vacuum leaks.
Check all fluid levels and let me know if any is below level or requires constant top-up.
