radiator does not flow proper while engine is running with cap off, but it pulsates up and down on the inside?

When the thermostat is closed, the coolant would not flow. When the radiator cap is not closed, pulsation would occur unless the thermostat is opened. System operates under pressure so the radiator cap has to be closed.

If you do not have overheating issues, there is nothing to worry about.
