1995 Chevrolet Silverado Clutch Went Out Again After 2 Years Already Rebiult

my clutch went out again after 2 years , I have already rebiult the transmission, my throwout bearing went out, I put a new clutch in and throw out bearing, I didnot replace the pilot bearing, It was making a loud squealing noise, when i opened it up the throwout bearing was toast and the fork was also, I put it all back together and it is still making the squealing noise louder than before, could it be the pilot bearing? I have to take it all back out again,to replace the pilot bearing, could it be my transmission? needs to be rebuilt?

I would do the pilot bearing and while the box is out feel for any problem with the front bearing in the box, when do you hear the noise exactly when does it start and stop
