Why doesn't my Ford Escort LX Wagon 1.9L 1994 (manual) want to shift into first gear?

Even at a complete stop and in neutral, it will not shift into first gear (when the clutch is pressed in all the way). The only way I have been able to get it to shift into first gear is if I shift into second gear then into first gear. It also makes a grinding noise when I let off the gas and I am still in gear (any gear), but when I shift into neutral, the sound goes away. It just recently started making this grinding noise.

A common cause of the gear shift problem mentioned is that the clutch is not opening up enough. The clutch pedal is not pushed down enough, or the clutch hydraulics are failing, either the slave cylinder or the master cylinder have leaks, or both are defective.
