I am having problems with my truck. What does the error code 998 hard fault present mean? It also said voltage on throttle position sensor was low.

Code 998 indicates a hard fault present. This is the dreaded "limp-mode". Extensive problems are what normally trip this code. Additional codes are usually present. Check the wiring to the TPS for damage or corrosion. What happens is when there is no signal from a sensor such as the TPS, the computer (FMEM) failure mode effect management over rides the sensor. Thus you get the dreaded 998 code. Since there is a problem with the TPS, that is most likely what has set this code.

TPS is the throttle position sensor. The TPS lets your computer know the position of the gas pedal and then uses this information to figure out the exact correct mixture of fuel and air needed.
