died and found distributor gear teeth almost gone what is problem?

93 ford aerostar already changed distributor and problem repeated. visual inspected camshaft and valve train components, found no mechanical damage.

I remember that happening to fords in the 80' and early 90's. there was a fix that used a flat washer under the distributor to shim it up to change the gear contact with the cam. I will try to find the tsb and get it to you.
