how do i replace the upper door hinge,on the driver side of a 1991 ford explorer

i can't get to nut on the inside of the door jam i have even taken the intersment cluster out of the dash and i still can't get to it.please help me if you can

Carefully. You need a wrench. You may not be able to get the wrench all the way onto the nuts or bolts. Our reference doesn't show this procedure, so unfortunately, I can't send you any images.
