1988 Chrysler 5th Ave hesitates on take off

When i drive my 88 Chrysler Fifth Avenue It slightly hesistates to take off then darts forward the other thing is that sometimes when i come to a complete stop it will just cut out and die for no apparent reason. What is going on?

Cuts out and dies when I come to a Complete stop unless I tap the gas. (Very Wasteful)

Hesistates to take off, but when it does shoots forward.

Which engine do you have? Is it the front or rear-wheel-drive model? They made both in '88 and '89.
