Front fan is hitting engine

The front fan in my my 1986 mercedes 300e has come lose and is hitting the engine. What could have caused this and how much should it cost to repair

The most common reason would be a failed fan clutch but without looking couldn't tell for sure. The fan clutch from the dealer is 169 plus tax labor is 1hr time's shop labor rate 80-100 per hr?If the fan blade was damaged that's 51 plus tax from the dealer. There could be some other damage but that's a round about price.
