Does that usually indicate an injector issue?

Hi:

Since the two are specific to cylinder 6, yes, there is a good chance it is a faulty injector. However, it could be a wiring or connector issue as well.

Do you have a constant misfire? If it is intermittent, try using a quality fuel injector cleaner in the fuel to see if that helps.

If that doesn't resolve the issue, then the injector itself needs to be checked. Here is a link that explains how to test an injector.

If you look below, pic 1 shows the cylinder orientation on this engine. The remaining pics are for removing fuel injectors.

In this case, the upper intake manifold needs removed to access the number 6 injector.

See pics below Note: Make sure to replace the o-rings on each injector and lubricate them with something like vasoline before reinstalling them.
