The common cause is the oil filter is "double-gasketed". The old o-ring gasket from the old oil filter sticks to the engine when the filter is removed. If no one sees it, the new gasket on the new filter goes over the top and you have two gaskets. There's a ridge on the filter to keep one gasket from squirting out but nothing to hold the old one. Oil pressure will blow the old gasket out and you'll have a major leak.
