Juddering often when start from cold

But not every time e.g. Can start engine and sit there idle and juddering will start or it starts when I engage gear. I have left the car with a Ford dealer twice for two days each time and they have been unable to recreate or find the fault. Any ideas please? It sort of feels like the blood (fuel) is trying to pump through the veins, but it's not smooth. Thanks
