The trims are all over the place. In the second set of pics (warm) the STFT is okay, but the LTFT is indicating a rich mixture. Have you tried cleaning the MAF or servicing the TB?
Also, in the first pics, was that when the engine was first started cold? I noticed the ECT indicates 122 degrees, and the system is running in a closed loop. The +38% STFT is extreme. That indicates the ECM is dumping fuel due to a lean mixture, which is the opposite of what I would have expected. With your scan tools, watch oxygen sensor voltages to see where they are.

Try servicing the MAF to see if that changes thing. Also, after you service it, delete the codes prior to starting it. See if they return or if something different comes back.
