Would you say the engine is surging and only when it is warm, that is the engine is warmed?
If so, my question that might seem stupid is when the engine is cold does it idle fine?
First thing to check is the engine coolant level. Then warm the engine and once it is good and warm, slowly crack open a bleeder on the engine coolant system. There might be some air in the system. Next look for vacuum leaks.
Is the check engine light on at all?
