High and rising idle

When you start the car the idle is a little high as normal cold start. When warm it usually kicks down to normal idle but it didn't, it starts to steady climb. I have change the throttle positioning sensor take out the EGR valve cleaned it to get the IAC cleaned it. I do have a couple of end of the vacuum that are not hooked up I don't know what those that would make that did that or not. Can you help me?
