Transmission problem
1991 Ford F150 V8 Two Wheel Drive Automatic

put the shift lever in drive, trans is in neutral, shift down to 1st and back up thru gears, will go into drive and shift thru all gears. Is there an adjustment needed? Or lever on side of trans needs replacing?

Has the check engine light come on? Is the trans fluid full? It could have to do with the trans control module (TCM).

Fluid is full but old, engine light is not on.

You may want to try a service (with a new filter); however, it may not change anything. My guess is there is a problem internally (valve body). If the TCM is the cause, I would think the check engine light would have come on. Try the service. It's the cheapest thing to do at this point.
