Trainsmission Bumps

The problem is the whenever I am driving, pulling up to 2K RPM(30MPH), and when the car changes its gears, I always feel a bump or a thud coming from the engine, I cannot determined if it's the transmission or the engine, I am thinking the transmission. Same thing while slowing down as well. The same bump also happens when I change from R to D, or from D to R. It never did this before. I don't want it to die on me. The car has about 140,059K miles as of today, and I hope I can take care of it. Thanks

My blazer jerks a little bit from 1st and 2nd gear and then from then on everything is somoth. I may do it for a week or two and then engine light goes off and the does not happen for several months, and the engine light is off. What could be the problem,
what parts would I need to replace in the transmission

P.S. The engine does not rev up, it is smooth after the bump.
