the intake heater only supplies warm air after the truck has been started, it is connect through the glow plug relay, most likely you had some burned-out glow plug which will not send power to the intake heater and the starting fluid did not help, using starting fluid on a glow plug system will most definitely burn out the glow plugs, check and replace the burned-out glow plugs and then check and see if you have power to the intake heater.
