Developed shifting issues particularly with third and fourth gears?

I have the vehicle listed above GLX 4motion 2.8L V6 with 5sp Auto Transmission (ZF-5hp19). It has developed shifting issues particularly with 3 and 4 gear. It hesitates shifting into either and if I accelerate, I may get a thunk noise and the car jerks. If I don’t accelerate to much it hesitates but will finally go into gear. I have found that if I use manual mode the shifting is better but not perfect. It still hesitates. I currently have code p0730 incorrect gear ratio. Done so far: replaced fluids and filter, replaced shift solenoids with remanuf, replaced transmission pressure regulator, replaced TCM with another used one. Things did improve after these changes but still having the issues, especially in auto mode. Codes prior to repairs were p0741 torque conv clutch circuit perform or stuck open, 17125-035 description same as p0741, 18265-035 Load signal error message from ecu, 18010-035 power supply terminal 30 voltage to low, 16486-035 mass airflow sensor signal too low, 17938-035 camshaft timing adjustment short to ground, p0444 evap purge control valve circuit open. All of these are gone but now have the p0730. The O2 sensors need replaced but I don’t think they are affecting the trans issue. I’m at a loss as to what is next. Is the transmission just shot? I’ve thought about getting another used one. Any help would be greatly appreciated.

The first thing I would do is check for leaks between the throttle body and the MAF sensor which can be causing the issue, if no leaks are found replace the MAF sensor. Next I would swap out the input shaft speed sensor because it sounds like it is weak.

P0730 - Incorrect Gear Ratio
P0741 - Torque Converter Clutch Circuit Performance or Stuck Off
P0444 - Evaporative Emission Control System Purge Control Valve Circuit Open

Please double check the transmission fluid level and condition to make sure it is not burnt, if so then the transmission will need to be rebuilt. Check out the images (below). Please let us know what happens.
