Had the same problem on my 1990 fifth avenue. Turned out to be damage to the headlight motor from wear and tear. Assuming you can hear the motor running and its getting power. There are 3 plastic round balls inside the motor assemply that could be cracked and broken. I ordered a new set from someone on ebay for maybe 5 bucks and disassembled the motor and repaired it and reinstalled it
