When I take off, as I start to move the truck jumps. It feels like someone just ran into the back. It feels like it comes from the back end. What is the problem and how urgent is it? Can I fix this myself?

Does it also make a "squawk" or "thump" noise when this happens?

Yes it did make a thump. However it is running fine now. I asked around town and shoulder shrugs was a typical response. One guy said that if the truck was running hot -( I had been driving almost all day)- then that could have been the problem. He suggested transmission fluid change and use an addative LUCAS OIL for transmissions. Your thoughts.

Get some nickel anti-seize from your local parts store.

Remove the rear driveshaft from the truck.
Brush a good amount of the anti-seize into the driveshaft yoke splines.
Install the driveshaft. (Make sure to slide it several times on the transfer case output shaft)
Drive the truck to verify the problem is fixed.

With the description you are giving me, It sounds like the driveshaft yoke is binding on the output shaft.
