Jerkie 98 S 10

My 98 s10 pick up has roughly 131,000 miles and it's seen better days. When I turn it on in the morning, the engine seems to jerk...and when I start driving it...the entire truck jerks the second I've switched gears. Then the Check Engine light turns on. However, if I drive it continuously for 1 day...the check engine light turns off I've had the entire exhaust system replaced but I know that isn't the problem. I replaced the fuel pump, and I'm going to get the filter replaced(cuz I dunno if it's ever been replaced) I maintain it pretty well, but do you guys have any ideas as to what the problem is?

...you can notice the truck jerk when your not in it. It usually jerks just after I have shifted gears. It kinda looks like the type of jerk when someone is first learning how to drive manually. PLus I heard about how the 98 S10's seem to have some sort of malfunction with the fuel injectors. That could be it too....Either way, I'd have to get someone qualified to do that work. I don't have the experience to do in depth stuff. I'm just wondering if anyone can possibly lead me to the right direction
