Check engine light is on cooling fan not working

I was driving home and my check engine light came on but no difference in running. When I got home the fan was running with the A/C on. I turned the A/C off in the driveway and ran in the house with the van still running and came back outside the fan was off and overheating. It has one electric fan with 4 wires going to it and only works with the A/C on. What could be the problem? And reverse lights still work.

We need to start with checking what the code is. More then likely this is something to do with the coolant temp sensor or PCM. However, we need to start with this. Based on the fact that the fan only runs with the A/C on, shows the PCM is not commanding it on when the engine gets hot.

This means we need to find out what the code is and go from there. I attached the testing for this issue and a guide on how to get codes. Thanks
