Belt will stay on for a little bit but pops back off

Already replaced belt and tension pulley.

More than likely you have a pulley that is either wobbling or not lined up. Have you changed any other pulleys recently or did this just start happening?

What I would do is install a new belt or the newest belt if it is in good condition and what the pulleys as the engine runs. You should be able to look down the belt and see all the pulleys running in line and nothing look like it is sticking out. If you do then that is the issue.

If you are not sure, can you get a video of the belt running and we will see if we can try and identify it.
