2002 Toyota Prius Transmission lock up

The transmission will lock up in about 40 --50 feet of run forward. goes ok in reverse... Dealer said just has a generic code. but no information from trans??

have a trans guy check this it may be two clutches locking up when they aren't supposed to be.
