2002 Mercury Cougar stalling issue

I have an 02 mercury cougar manual, it keeps stalling when slowing down or stopped at a stop light. It has done this in the past and had it fixed twice there were holes in one of the hoses or lines. Fixed the problem then it comes back after a few months. I recently also replaced the Idle air control valve because it went out but I think I started to hear it making the same noise it did when it went out last time. The check engine light is on and says bank 1 & 2 running lean. What could the issues be. If its more hoses with holes what would keep making them get holes? Just old and break could be it but would anything else cause them to do that?

If you have a lean fuel mixture, I would believe that you have a vacuum leak causing the issue. Check all vacuum hoses for damage. Also, check the intake gasket for leaks.
