Why has my car got a distinguishable rough idle?

So far i've replaced the distributor, the spark plug leads and the spark plugs. I've also cleaned the throttle body. It also used to surge right after it started and it evens out after about 5 seconds but it stopped doing that after I cleaned the throttle body

Oh mistake I made it has 136000 miles on the clock not 220 000
