I have a 97 dodge grand caravan. The serpentine belt came off and caused it to slip time. Had that repaired. Also replaced Idle Air Control Valve, upper intake gasket, and distributor cap. It will crank and run fine in idle, but when you put it in gear it dies. If you double foot it will go.

Also, what would cause my serpentine belt to keep coming off after all pulleys have been replaced?

first is ti the timing belt or the belt for alternator. If that keeps coming off then ther eis misalignment someplace. was the tensioner replaced as well? it should have been. Is this a free spin motor? if the timing belt was replaced and it's not a free spin motor you mayhave bent valves.
