My Mazda 626 1993 won't shift out for the third time

Wen I drive it will shift out two times. The third it will sound as if it's going to but never do.

It sounds like OD is not working. 1-2 2-3 3-4, 4 being OD.

It shifts in 1st gear at 10 miles and 2nd gear at 35 miles

What rpms are you turning at what speed when it does not shift.

For what its worth, i've never seen a mazda 626 make it more than 75K on one tranny. And usually the replacement goes about 35K later. What color is the transmission fluid? If it's not red, trade it or put a transmission in it. Sorry for the negative prognosis.
