tension pulley

belt keeps slipping off of tension pulley

On of the pulley will be out of alignment, normally due to a bad bearing, also check that the front engine pulley is ok, there is no end movement and if it is a rubber type harmonic balancer that the outer section has not started to detached from the hub.
