1995 Nissan Sentra nO. 3 PLUG NOT FIRING

I HAVE A '97 ENGINE 1.6L in my '95 sentra. New plugs and wires recently new cap and rotor. The number three plug that I pulled out was clogged with black build up. The new one has been in for three months and shows no sign of build up or detonation. It looks like new whereas the other three are clearly firing correctly. 20mpg not the 30 I should be getting. Rich smelling exhaust, and an obvious odor of fuel in the oil when I change it.

Is there any engine codes showing up? If so, scan for codes and post so I can better diagnose the problem.

If no codes are present, perform following tests on components.
